WHAT DO I NEED TO DO?
You MUST bring proof of address & Photo ID EVERY VISIT so that they know you are a Noble County resident who lives in the 46755 zip code.
Lease agreement
Current utility bill in the name of the person living in the home
Piece of mail addressed to you with the last 4 weeks


They do not accept check stubs, rent receipts or photo ID as proof of address.



Bring a current photo ID for all adults 18 and older.




For children under the age of 18, you must provide one of the following:



Birth Certificate
Social Security Card
Passport
Legal Documents
Medicaid card with the individual's name



School Social Worker letter stating the child is in the household





HOW DOES IT WORK?



When you get there, you'll get a ticket number.


Keep half for yourself and place the other half in the jar on the receptionist's desk for a lottery style order.



FIRST VISIT:
Complete the application while you wait for your number to be called.


You must present your photo ID AND Proof of Address (You will need this Every time you come to shop.)


You must also provide ID for ALL other members of the household.


If you come without the ID's on your first visit, you will be allowed to shop only for the members of the household for whom you have presented ID's that day.


If the address on your photo ID does not match your proof of address, you may shop on your first visit only.


You will not be allowed to shop again until ID's for ALL members of the household are presented AND ALL photo ID's in the household match your proof of address.



ALL VISITS AFTER THE FIRST VISIT:
All ID's must be on file with them for EVERY member of the household.



The address on ALL Photo ID's MUST MATCH your proof of address.


Once all ID's are updated and copied in their file, you are required to bring a current photo ID AND proof of address for the person shopping EVERY time you shop.


You may shop twice per month.


When your number is called, you will be given an item number card.


That is the number of items you are allowed for your family size.


You will take a cart and a volunteer will help you through the food center while you shop until you get the hang of things.



Once you have all of your items, volunteers will count your items and weigh them for their records.


Then you will be on your way!



Conditions that may result in no longer receiving services from friendshio connection or any ministry of common Grace Ministries:


1. Being dishonest on the application or at any time during your relationship with any ministry of common grace ministries.


2.Being disrespectful to staff, volunteers or other NEIGHBORS
